We love seeing you and your pets, but you don’t always need to come in for an exam to get the advice and answers you need. Follow-up questions about a recent visit (within the last week or so) are complimentary, but when there are new issues or more time has passed Access Vet Care offers convenient Virtual Consults ($39-54), as well as a Case Review ($28) option when you have more basic questions about your pet.

This form will help you get the consultation you need, and sort out what kind of consult best fits your needs if you aren’t sure!

Are you concerned this may be an emergency? *
Virtual Consults or Case Review are NOT appropriate for potential emergencies. Please call AVC at 812-590-3081 if you are a current client and this is during business hours, or call Jefferson Animal Hospital or Metropolitan Veterinary Specialists after hours or on weekends.

Follow-Up: We want to make sure you leave each visit with an understanding of your pet’s issues and our recommendations. If you have questions regarding issues your pet was seen for, diagnostics, or treatments from a visit within the last week we are here to answer. We appreciate you submitting those questions with this form, as it allows us to follow up efficiently and accurately.

Note that new issues since your visit, or questions beyond a week, will need a Case Review ($26), Virtual Visit ($54), or standard in-clinic appointment as appropriate to the nature of your concern.

Is this a follow-up question about issues discussed at your recent visit, or about treatments or diagnostics from that visit?

New issues will require a Case Review or Consult so the Veterinarian can devote the time needed to them. Please select below.

Have we seen at least one of the pets involved within the last 8-10 months?
Telemedicine Visits and Case Review options are only available for pets that have been examined by an AVC Veterinarian within the last year. Please call or submit an appointment request to schedule an exam.

Case Review or Virtual Consult

Which best describes your needs:
Are you concerned about aggression towards people or other pets?
STOP:  Case Review is not appropriate for aggression concerns; you will need to consult at length with the doctor before we can consider any intervention. Select Virtual Consult above to schedule if you’d prefer not to bring your pet to the office at this time, or call to schedule an in-clinic exam and consult.

You will not be able to submit this form if you continue with this option.

Is this Virtual Consult:

Case Review: The Veterinarian will review your patient’s medical history and the information you provide, then email or have the staff reach out with an answer to a straightforward question. This is only appropriate for simple, specific questions the Doc can answer quickly. Common questions include:

  • I noticed this new or recurrent concern, do you think my pet needs to come in for an exam? (photos or video can be submitted)
  • Follow-up questions about something my pet was seen for more than a week ago.
  • A general question about caring for pet(s) that I’d like the Doctor’s advice on.

Virtual Consult: We’ll schedule a block of time for the Veterinarian to meet with you directly via Google Meet or telephone. You’ll provide us as much info as possible using this form so the vet can review it prior to your consult and use your time well.

This is identical to an in-office visit, minus bringing your pet in for an exam. It’s ideal for:

  • Follow-up on many ongoing or chronic issues.
  • Many behavioral concerns.
  • Complicated questions about your pet’s health that require more time and focus from the Doctor
  • Starting the conversation and making clear plans for follow up when you can’t get your pet to the clinic right away.

If you know what you need, select it above. If you’re still not sure, keep answering the questions below to help decide. Also remember that if you’re still just not sure what’s best, it’s never wrong to just schedule an old-fashioned In-Clinic Visit with the Vet!

Is this a specific question about a medical or behavioral concern with your pet(s)?
If this is a general question and isn’t urgent, consider posting it on our Facebook Page rather than paying for case review. We stop by every few days to answer all these questions. You might browse a bit, as someone else may have had a similar question before! You should also try the search function on our website, or browse the Pet Resources section where you’ll find tons of informational articles.

If you’d like a faster response, you’re welcome to continue submitting a Case Review for the usual fee – you’ll need to change your answer above to proceed.

If you have multiple or complex questions for the Veterinarian, either a Virtual Visit or in-clinic appointment will be necessary to ensure to provide the Doctor time and focus to address your concerns appropriately.

Please go back and choose Virtual Visit, or Schedule Appointment, to continue.

Are these issues:
If you have multiple or complex questions for the Veterinarian, either a Virtual Visit or in-clinic appointment will be necessary to ensure to provide the Doctor time and focus to address your concerns appropriately. An in-clinic visit is usually best when possible

Please go back and choose Virtual Consult, or Schedule Appointment, to continue.

What best describes the NEW concern/issue? *
Please return to the top and select Case Review to proceed.
How many times has your pet been seen at AVC for this issue
Even if this issue is ongoing, if it’s new to us at AVC either a Virtual or In-Clinic Consult will be required, as extensive review of past history is necessary to advise on ongoing problems. Please note that the majority of the time the Doctor will require an exam prior to initiating or changing treatments with such issues, so while we can always start with a Virtual Consult you’re usually better served to just schedule an In-Clinic Consult when possible.

Please return above and select Virtual Consult or In-Clinic Consult.

*If* your concern is limited to one very specific question, Case Review may be appropriate, but if we’ve only seen your pet once or twice for this issue do be aware that the doctor may still require a follow-up exam or Virtual Consult to ensure the best treatment and advice for your pet.
What best describes your needs regarding the CHRONIC or RECURRENT issue?
That’s beyond the scope of Case Review. Please select Virtual Consult above to schedule time to discuss these concerns with the doctor.
Case Review is appropriate for straightforward questions about recurrent issues or minor changes to ongoing issues. Return to the top and select that to proceed.

Note: If you have multiple or more complicated questions, or if you’d like to speak one-on-one about this with the doctor for any reason, you should setup a Virtual Consult instead.

That’s easy – if the recurrence is substantially different or worse than previously, an In-Clinic Consult & Exam is always recommended.

Depending on what’s going on an exam may not be urgent, and you can always start with a Virtual Consult if that’s easier for you. Just be aware the doctor will almost certainly recommend we follow up with an exam in the immediate to near future, and starting with an In-Clinic Consult & Exam might save you a bit in the long run.

Please return to the top and select Virtual Consult or In-Clinic Exam & Consult.
